Për këtë kontratë nuk është paraqitur Certifikata e marrjes në dorëzim dhe Akti i kolaudimit i cili është kusht që të shoqërohët me Shtojcën 9 Formularin e vlerësimit pasi vetë formulari nuk është i vlefshëm nëse nuk shoqërohet me Akt kolaudimi dhe situacionet .
